Their interaction with the public makes it critical we offer them access to vaccines – at any age.

The new governor's order means health care workers are eligible, and they now have the supply of vaccines that we can set aside for this group.

Here is who is included in this group:

  • nurses
  • home health providers
  • home medical equipment providers
  • therapists
  • dental office staff
  • pharmacists
  • administrative/reception staff
  • physicians
  • paramedics
  • emergency medical technicians
  • phlebotomists

What you will need to bring with you:

  • Organizational identification card
  • Medical license
  • Paystub showing employment at a health care organization
  • W-2 IRS form showing the health care organization as the employer
  • An employment verification letter on official letterhead

  • Create an account with CDR Health Pro by visiting patientportalfl.com. You cannot make an appointment without first creating an account.
  • Once you create an account, there will be an option for health care workers to enter a scheduling code. The scheduling code is RADICE03. Once you enter the code, you will see available appointment times for the Ed Radice Sports Complex.
